Northside Behavioral Health Center has an immediate opening for a Targeted Case Manager. Job Summary:

Provides case management services for adults experiencing serious and persistent mental illness by going into homes, work sites or other treatment settings. Responsible for assessment, planning, linking, monitoring and advocacy for the most appropriate mental health services for each client. Effectively maintains and manages assigned caseload, contingent on needs, strengths, abilities, barriers, and preferences of the individual served. Travel and transporting clients is required. Position Requirements:

This position requires a valid Florida Driver's License with a 3-year clean driving record in Florida.

Required Experience/Education:

• Bachelor Degree and at least 1 year of full-time case management experience with Adult Severe and Persistent Mental Illness (SPMI) population.
  
  
• Florida Certification Board Certified Behavioral Health Case Manager (CBHCM) credential is preferred, or will be required to be obtained within the first year of employment.

Job Type: Full-time

Pay: $35,360.00 - $37,000.00 per year

• 8 hour shift
