• JOB SUMMARY
  
  
  • Working in conjunction with the Administrative Director of Nursing, Director of Nursing, RN and LPN, the CNA performs a variety of basic resident care activities in caring for the personal needs and comfort of residents. The CNA works under the direction of a licensed nurse who must follow the principles of delegation. CNA work is reflective of CCH mission, vision and values. The CNA will integrate departmental goals for improvement across the care continuum. The CNA practices within the framework of the Wyoming Nurse Practice Act, Wyoming Department of Health and CMS guidelines.
    
    
• PRIMARY JOB DUTIES
  
  
  • Assists with basic resident care and ADL’s according to plan of care.
    
    
  • Observes residents and reports unusual conditions and/or responses to Charge Nurse of respective Unit.
    
    
  • Enhances resident safety by attending bedside shift huddle and performance of hourly rounding.
    
    
  • Assists with admission, discharge, and transfer of residents.
    
    
  • Assists nutrition staff in passing meal trays and setup per care plan.
    
    
  • Passes water, ice, clean linens and supplies to resident/patient rooms.
    
    
  • Fills Helios portable oxygen machines utilizing appropriate PPE.
    
    
  • Maintains a clean and safe environment.
    
    
  • Performs routine cleaning tasks.
    
    
  • Provides service excellence to all customers in accordance with AIDET, Excellence Every Day and Standards of behavior.
    
    
  • Demonstrates efficient time management and prioritization skills.
    
    
  • Documents on residents according to policy; ADLs, Behavior, Bath and Skin integrity and etc.
    
    
  • Practices fiscal responsibility and accountability.
    
    
  • Actively participates in facility committees and Quality Improvement Program.
    
    
  • Alerts staff when supplies, including equipment are needed and replenishes stock.
    
    
  • Completes van competency and transports residents as needed.
    
    
  • May be required to work on other nursing units according to distribution of staff and residents.
    
    
  • Adheres to CCH policies and procedures.
    
    
  • Complies with the hospital’s Corporate Compliance Program including, but not limited to, the Code of Conduct, laws and regulations, and hospital policies and procedures.
    
    
  • Must be free from governmental sanctions involving health care and/or financial practices.
    
    
  • Performs other duties as assigned.
    
    
  • Responds to mental health needs.
    
    
• JOB SPECIFICATIONS
  
  
  • Education: Graduate of an state or board approved training program
    
    
  • Licensure: Current unencumbered Wyoming CNA license and must register with Wyoming Department of Health CNA Abuse Registry
    
    
  • Certifications required: See Cardiopulmonary Resuscitation Certification Policy and Certifications/Education Requirements Policy.
    
    
  • Experience: Experience in long-term care/skilled nursing preferred.
    
    
  • Essential Technical/Motor Skills: Hand/eye coordination in order to take vital signs, operate medical equipment, and use computer, etc. Speaking clearly to communicate with residents, families, physicians and staff regarding patient condition and care.
    
    
  • Interpersonal Skills: Ability to develop positive interaction with residents, families, physicians and staff in order to effectively care for the residents.
    
    
  • Essential Physical Requirements: Ability to lift and/or maneuver objects weighing 40 pounds or less in the assessment and implementation of resident care. Requires standing or walking for long periods of time. Requires stooping, bending, pushing and pulling.
    
    
  • Essential Mental Abilities: Must be able to patient’s condition, formulate a plan of care, select appropriate interventions, evaluate patient response to care/treatment and to explain/teach patients about their condition/recovery. Requires higher level of mental faculties accompanied by short- and long-term memory. Ability to function in stressful situations.
    
    
  • Essential Sensory Requirements: Corrected vision, hearing to normal range.
    
    
  • Exposure to Hazards: OSHA CATEGORY I. TASK THAT INVOLVES EXPOSURE TO BLOOD, BODY FLUIDS, OR TISSUES. All procedures or other job-related tasks that involve an inherent potential for mucous membrane or skin contact with blood, body fluids, or tissues, or potential for spills or splashes of them, are Category I Tasks. Use of appropriate protective measures is required for every employee engaged in Category I Tasks.
    
    
  • Hours of Work: Primarily 12 hour shifts dependent on demands of unit. Rotating weekends and holidays. May be required to work up to 48 hours dependent on demands of unit.
