LPN Responsibilities:

• Provide direct care and treatment to individuals with DD to meet their daily physical, mental, educational and spiritual needs
  
  
• Carry out the required nursing functions as well as assist direct care staff in performing daily living activities and programming for the residents
  
  
• Responsible for the daily medical documentation
  
  
• Coordinate residents medical needs internally and externally
  
  
• Assist with medical appointments and diagnostic testing
  
  
• Provides a daily link between all internal and external providers
  
  
• Check and monitor vital signs and weights
  
  
• Perform direct care needs such as dressing changes, tube feeds, IVs, distribution of medications and general care

LPN - KNOWLEDGE, SKILLS AND ABILITIES:

• LPN must have knowledge of federal, state, and local regulations regarding the medical care and treatment of ID/DD individuals
  
  
• LPN must have comprehensive knowledge of the Nurse Practice Act
  
  
• LPN must have the ability to transport clients using corporate vehicles for medical appointments
  
  
• LPN must have ability to supervise and direct the workflow of direct care staff
  
  
• LPN will be working with ID/DD clients of varying functionality in a professional manner
  
  
• LPN must have comprehensive knowledge in Universal Precautions, Infection Control, Patient Rights, Safety, and Building Evacuations
  
  
• LPN must have the ability to deal tactfully and courteously with regulators, co-workers, MR/DD clients and family

LPN - QUALIFICATIONS:

• Must be a licensed LPN for minimum of 1 year
• Experience working with ID/DD individuals
• Requires a valid driver’s license in accordance with EHVI policy
• Basic computer knowledge of Microsoft Office Products

• Nursing: 1 year
