SUMMARY
The Licensed Practical Nurse (LPN) Telehealth Nurse provides patient care based on their state board of nursing “scope of practice”. The LVN/LPN provides nursing care to patients as part of the multidisciplinary team in accordance with the hospital’s mission, physician and patient/s expectations. The Licensed Vocational Nurse/Licensed Practical Nurse shall be responsible and accountable for making decisions based upon the individual’s educational preparation and experience in nursing.
Hours
6:00 p.m. to 6:00 a.m. Three nights per week, every other weekend and rotating holidays.
PRIMARY RESPONSIBILITIES

1.Performs nursing care for assigned patients within the limitations of licensure and nursing department policy and licensure.
2. Implements nursing actions to achieve patient care goals.
3. Participate in the evaluation of quality of patient care and outcomes based on care and treatments administered.
4. Assists the patient care team with formulating the plan of care and updates according to policy.
5. Participates as a member of the Patient Care Team that includes: Virtual Care RN, bedside RN, and C.N.A.
6. Accepts responsibility for one’s own professional standards.
KNOWLEDGE AND SKILLS REQUIRED:
EDUCATION
Education: Graduate of an accredited school of nursing with a Licensed Practical Nurse (LPN) required.
Experience:1-2 years clinical experience in focus area preferred.
License/Certifications: Current Nebraska Licensed Practice Nurse license required. Current Licensed Practical Nurse license required in state where the patient is located will be required immediately upon hire. Experience in a supervisory or quality assurance position in acute and/or home health, preferred.
Special qualifications: Manage and communicate data in a nursing informatics work environment to Managed Services Clients, nurses, health care providers and patients. Act as the liaison between nurses and technology people. Must be able to successfully complete established competencies for the position within designated probationary period. Effective communication skills; both orally and written. Computer skills needed. Ability to work with others within a team to ensure quality patient care. Strong critical problem-solving skills. Ability to respond to common inquiries or complaints from customers or regulatory agencies. Ability to define problems, collect data, establish facts and draw valid conclusions. Ability to work independently and under pressure.
Hours of work: PRN, Part-Time and/or Full-Time Non-Exempt – 12-hour shift, rotating weekends.
