Children's Health Home Care Manager – starting salary of $38,500.

Additional Details:

The CHHCM position is considered an essential position, which includes fieldwork(home/community visiting) -- 50% fieldwork with children and families; and 50% in the office doing documentation, paperwork, planning, team meetings, training, etc.

Will be providing services that help to see better health outcomes for children who have chronic medical or behavioral health conditions.

Kids being serviced are ages birth through 21; the bulk of the referrals are coming from foster care.

The ideal candidate will have some significant work experience with children and families; and should be able to talk about this and give relevant examples.

Should have experience with and an understanding of home visiting.

Should understand both the education piece and the social determinant piece.

This CHHCM position is a Care Manager Level III –

There are three different acuity levels for the cases/kids Berkshire services -- Some organizations hire Care Managers at either a Level I, Level II, or Level III, and are only assigned to handle cases at the comparable level. (For example, a Care Manager Level IICANNOT take on Level III cases.) Berkshire only hires for CM Level III so that the team can service cases at all acuity levels.

There's a heavy Medicaid compliance with the program, so must be strong with documentation, have an understanding of regulatory compliance, and have very good time management skills.

Should be familiar with EHR (electronic health records) and be able to get assessments and reassessments done in a timely manner to adhere to compliance requirements (again, must be a strong time manager).

Primarily the work is scheduled for and being done Monday through Friday 9 to 5, but the employee must be able to be flexible with their time and schedule to meet the needs and availability of kids and families. There will be occasional earlier morning, after hours/evening, or weekend appointments to accommodate clients’ schedules.
